Grant County Community Health Needs Assessment

Under the Patient Protection and Affordable Care Act (PPACA), tax exempt hospitals such as Blue Mountain Hospital are required to conduct a Community Health Needs Assessment (CHNA) every three years. This involves getting input from our community members, public health experts and key stakeholders.

The CHNA identifies health issues facing our community. Utilizing this information, a plan is then developed with community partners to address the priority health needs.
